Gentiva Health Services is a United States end of life and home based care provider headquartered in Atlanta, Georgia. It delivers hospice, palliative, community and personal care to patients in their own homes, nursing facilities and inpatient units, operating through regional brands including Gentiva Hospice, Heartland Hospice, Heartland Home Health, Hospice Plus, New Century Hospice, SouthernCare, Regency SouthernCare and Empatia Palliative Care. The company was formed on 11 August 2022 when the hospice and personal care divisions of Kindred at Home were carved out and recapitalised with the private equity firm Clayton, Dubilier & Rice taking majority control. It has since grown by acquisition, notably buying ProMedica's home health and hospice business in November 2023, and runs more than 600 locations across 38 states with over 35,000 staff.
