The Register tab above is hand research: a dozen holders, each traced to the filing it came from. This asks the same question of the whole market for the quarter ended 31-mar-2026, where 4,309 managers reported a position. Broad where the research is deep, and shallow in the ways a 13F always is.

Built from 13F filings, which report US listed, long, US custodied positions only. A register from this source is a floor rather than a level: a holder's true position can be far larger, as BlackRock's ASML stake shows at 1.28m shares in a 13F against 26.3m in its 13G/A. Holdings held through derivatives, outside US custody, or below a manager's reporting obligation do not appear. Where a holder is marked as a family, its total sums several separate filing entities and describes something no single filing describes; the constituent entities and their share counts are listed so the total can be checked. That roll-up can legitimately exceed the family's own Schedule 13G, because a 13G is filed by one legal entity: Vanguard reports Alphabet through ten advisory entities, and The Vanguard Group Inc itself reported zero after its January 2026 disaggregation.
What the filings say against what the research found
Every holding above was also resolved directly from EDGAR through a precedence rule: the company's own filing outranks a 13D or 13G, which outranks a 13F, which outranks hand research. A 13F reports only US listed, long, US custodied positions, so it is a floor rather than a level. Where sources disagree the higher one leads and the lower is kept beside it.

What a reader needs to know to read these numbers
- Lilly Endowment, Inc. is the distinctive name on this register and it is not an index fund and not part of the company. It is a private charitable foundation, an Indiana not for profit corporation and a 501(c)(3) private foundation under section 509(a) of the Internal Revenue Code, with its office at 2801 North Meridian Street, Indianapolis. It was created in 1937 by members of the Lilly family, endowed with the family's Lilly stock, and it makes grants in religion, education and community development in Indiana. It is legally and operationally separate from Eli Lilly and Company: it holds sole voting and sole dispositive power over all 90,376,978 shares and it has no shares it merely has the right to acquire. Lilly's articles of incorporation explicitly carve the Endowment out of the definition of a Related Person for the company's anti takeover article, so its 9.6 percent block does not trigger those provisions. The Endowment sells Lilly stock over time to fund grants, which is why its stake has drifted down: 16.3 percent in its 1994 Schedule 13G, 9.8 percent at 31 December 2025, 9.6 percent at 30 June 2026.
- Eli Lilly and Company files no Form 13F. Its full EDGAR submission history at data.sec.gov contains no filing of any 13F type, so the register of its own holdings has to be assembled from the Schedules 13D and 13G that Lilly itself files as an owner, plus the aggregate investment lines in the 10-Q. The four positions above are all of the Lilly-as-filer 13D and 13G positions that are current as of August 2026.
- Balance sheet context for the size of the whole equity portfolio, from the 10-Q for the quarter ended 30 June 2026: marketable equity securities carried at fair value of $796m (up from $223m at 31 December 2025), equity investments without readily determinable fair values of $1,073m (up from $846m), and equity method investments of $1,537m (up from $1,288m). Lilly does not name the individual investees behind those totals, so the four named positions here account for only a part of roughly $3.4bn of equity investments.
- Disagreement on Lilly's own share count, recorded rather than resolved. The Form 10-Q cover page states 941,357,065 shares of common stock outstanding as of 3 August 2026, and the Endowment's Schedule 13G/A uses 941,741,406 as of 27 April 2026 taken from the Q1 10-Q. Stockanalysis.com on 14 August 2026 showed 891.36m shares and a market cap of $1.05tn. I have used the SEC filed count. The market cap here is therefore derived: 941,357,065 shares multiplied by the LLY close of $1,180.16 on 14 August 2026 equals $1,110,951,953,830. If the 891.36m count is right the figure would be about $1.05tn instead.
- Vanguard's position is split across two filings because of an internal reorganisation. On 12 January 2026 The Vanguard Group, Inc. disaggregated, and its Schedule 13G/A of 26 March 2026 now reports zero shares. The successor reporting entity, Vanguard Capital Management LLC and named affiliates, reports 61,295,605 shares or 6.48 percent as of 31 March 2026. Both rows are shown above so the older 80,952,595 share figure that Lilly's March 2026 proxy carries for The Vanguard Group as of 30 September 2025 is not silently dropped.
- The BlackRock figure is stale by design. Lilly's March 2026 proxy still sources BlackRock from a Schedule 13G/A filed 29 January 2024 reporting ownership as of 31 December 2023. I found no more recent BlackRock filing on LLY, so the as_of is 31 December 2023 and the 6.8 percent is against Lilly's 25 February 2026 share count, not the 2023 one.
- PNC's 5.4 percent is not an ordinary asset manager stake. 50,000,000 of PNC's 51,153,837 shares are held in the Eli Lilly and Company Compensation Trust, for which PNC Bank is the directed trustee, so most of that block is company benefit plan stock rather than PNC client money. That is why it is typed strategic rather than index. The proxy footnote does not give a measurement date for the underlying 13G/A filed 7 February 2025, so as_of is left null rather than guessed.
- Milestone and royalty obligations are not holdings and none are included here. Lilly's 2026 deals with Ajax Therapeutics (AJ1-11095, June 2026, $909m acquired IPR&D charge), Orna Therapeutics (ORN-252, May 2026, $1,233m) and Scorpion Therapeutics (STX-478, March 2025, $1,412m) were asset and licence acquisitions expensed as acquired in-process R&D, not equity stakes.
- Wholly owned acquisitions are excluded per the brief. In the first half of 2026 Lilly closed business combinations for Kelonia Therapeutics (25 June 2026, $4,897m consideration), Centessa Pharmaceuticals plc (24 June 2026, $6,593m) and Ventyx Biosciences (4 March 2026, $1,178m), and in July 2026 paid about $2.0bn for three infectious disease companies. It also has a pending agreement to acquire AtaiBeckley Inc. for approximately $2.8bn at closing. These are subsidiaries or pending subsidiaries, not minority holdings.
- One exit worth recording: Lilly and Eli Lilly S.A. filed a Schedule 13G/A on 3 October 2025 reporting zero shares of Immunocore Holdings plc as of 30 September 2025, closing out a position first reported in February 2022.
- Percentages in the register are percent of the single class of common stock, which is also percent of the company. Lilly has one class of common stock with one vote per share and no dual class structure, so there is no founder or family voting block. The Endowment's influence is economic, not structural.
- Values in the holdings rows are derived, not filed: filed share count multiplied by the target's closing price on 14 August 2026, the last trading day before the 16 August 2026 as-of date. Register rows carry no value because the holdings were measured on several different dates.
Looked for, not found
Recorded rather than filled in. Each of these is a place where a number could have been invented and was not.
- The individual investees behind the 10-Q's $1,073m of equity investments without readily determinable fair values and $1,537m of equity method investments. Lilly discloses only the aggregates in Note 7 of the 10-Q for the quarter ended 30 June 2026 and names no counterparties. This is where the private company stakes sit and it is the largest gap in this file.
- Whether Lilly still holds any of its older disclosed public positions. Lilly filed Schedules 13G on Foghorn Therapeutics (December 2021), Aeglea BioTherapeutics, Leap Therapeutics, Avidity Biosciences, Surface Oncology and Forma Therapeutics (all 10 February 2022), and 13G/As on AC Immune SA and Precision BioSciences (both 27 January 2023). None has been amended since, and several issuers have been acquired or restructured. I did not carry any of them because their last filed figures are three to four years stale and I could not confirm a current share count from a fetched page. I looked in the EDGAR submissions history for CIK 0000059478 and in each filing's index page.
- A current traded price and any exchange listing for Ionetix Corp. Its Schedule 13G carries a CUSIP of all zeros and I found no ticker on any page I fetched, so the 8,411,397 share position is left unvalued.
- State Street's position in LLY. It is below the 5 percent proxy disclosure threshold and I found no Schedule 13G from it on LLY in the EDGAR submissions history, so it is not included rather than estimated.
- A measurement date for PNC's 51,153,837 shares. Lilly's proxy cites the 13G/A filed 7 February 2025 without stating the as-of date, and I did not fetch that filing itself.
- A second independent confirmation of Lilly's shares outstanding to settle the 941.4m versus 891.4m discrepancy described in notes.
