Aflac Incorporated (American Family Life Assurance Company) is an American insurance company and is the largest provider of supplemental insurance in the United States. It was founded in 1955 and is based in Columbus, Georgia. In the U.S., it underwrites a wide range of insurance policies, but is perhaps more known for its payroll deduction insurance coverage, which pays cash benefits when a policyholder has a covered accident or illness. The company states it "provides financial protection to more than 50 million people worldwide".

What a reader needs to know to read these numbers
- Aflac's own 13F, and how it was treated. This tiny filing (4 lines, versus tens of thousands of rows for the institutional 13Fs used in the register below) is not a proxy for Aflac's whole investment portfolio, which totals well over $100 billion in fixed maturities and other investments and is overwhelmingly non-13(f) fixed income; it only captures the handful of US-exchange-listed equity positions Aflac itself holds with investment discretion, each tied to a disclosed distribution alliance or acquisition, so all three are recorded as holdings rather than waved off.

- Japan Post Holdings is the standout 'not just an index fund' register entry (see its method_note): a 10.15%, ~$6.0 billion strategic equity position held through a New York voting trust under a 2019 Shareholders Agreement tied to Aflac Japan's decades-long cancer-insurance distribution partnership through Japan Post's post office network (affiliates of Japan Post Holdings received approximately $102 million in commission and other payments from Aflac Japan and its affiliates in 2025, per the 2026 proxy's Related Person Transactions section).
